London North PCT commissioners within this scheme, clinical staff from both primary and secondary care, GPs, hospital consultants, nursing home representatives, discharge co-ordinators, coronary care staff, renal network clinicians, dieticians, cardiac network clinicians, and practice based commissioning leads/consortia. |
Clinical services being commissioned and covered within this workshop are:
Cardiac Rehabilitation, Pulmonary Rehabilitation, Renal Dialysis, Stroke Early Discharge, Acute Home Care, Elective Surgery, Sexual Health and Chemotherapy.

Clinicenta working in the South East
