How to Refer to the Service
Direct Referrals
All referral forms and letters should go directly to the Clinicenta Care Management Centrevia fax no: 0845 602 1903, or to your local PCT Referral Centre which will pass on your request.

You can refer via an nhs.net email having downloaded and completed the referral form and emailing it to cl.clinicenta@nhs.net.
